Praecipe and Docketing Statement
A form that is filed along with a notice of appeal that provides the parties, the appellate court, and the clerk of courts information about the appeal. This form must be filed with any notice of appeal.

Affidavit of Indigency
An affidavit that is required to be filed by any individual who is indigent and seeks a waiver of fees.
Mediation Statement Form
A form that is required to be completed and returned to the court mediator prior to a party’s scheduled mediation date. The form is kept confidential and is used to assist the mediator in facilitating a resolution.
Application to Serve as Appointed Counsel
A form that is required to be submitted if interested in being added to the court’s assigned counsel list. The form may also be used to renew service for those attorneys already on the assigned counsel list who desire to continue serving the next year.
Oral Argument Recording Request Form
A form that is required to be completed prior to obtaining a copy of an oral argument audio recording.
